How it works

We know that going to the emergency room or hospital when you get sick isn’t easy on you or your family. Neither is seeing your office physician when it may be difficult to find a ride or even leave your home. Care Level Management is pleased to introduce you to an additional option for care, the Personal Visiting Physician service, which provides you with round-the-clock house calls by a physician who treats you in the comfort and convenience of your own home.

Here’s how it works:

You will have round-the-clock access to a physician who can come to your home anytime he or she is needed. This helps prevent emergency room (ER) visits and unnecessary hospital stays.
You can continue to see your primary care physician as often as you need to. Your Care Level Management In-Home Physician works directly with your physician, and gives him or her regular updates on your condition.
You will receive your Care Level Management physician’s direct cell phone number. Anytime you feel ill, call immediately, 24 hours a day, 7 days a week, even on weekends and holidays.
A house call keeps you from having to find transportation and coordinate a doctor’s schedule when it is difficult for you to go out.
Working with each Care Level Management physician is a Nurse Care Manager, who helps arrange for the treatment and services prescribed, and coordinates your care with your other physicians.
